The Reddit column is a flat 100 to 600 mcg, taken at night. For an adult, the pediatric label figure is easy to turn into arithmetic: 30 mcg per kg for a 70 kg person would be 2,100 mcg. That is only arithmetic. The label was for children with a diagnosed deficiency, and I am not suggesting anyone scale it up.

I did not find a controlled trial of the 200 to 500 mcg nightly range that clinics and forums treat as standard, so that number rests on clinical habit. The common claim that 200 to 300 mcg is the labelled dose does not match the sources I could check.

What the FDA label and regulatory record say

I verified the regulatory facts in the Federal Register rather than from vendor pages. Sermorelin was sold in the United States as Geref, made by EMD Serono, under two applications. The 0.05 mg ampule was approved on December 28, 1990 to evaluate whether the pituitary can still secrete growth hormone. The 0.5 mg and 1 mg vials were approved on September 26, 1997 for idiopathic growth hormone deficiency in children with growth failure.

The company told FDA it was discontinuing the ampule in July 2008 and the vials in December 2008, and approval of both applications was withdrawn effective June 18, 2009. In a March 4, 2013 notice, FDA determined that neither product was withdrawn for reasons of safety or effectiveness. A DailyMed search on 2026-10-11 returned no sermorelin label.

Because there is no current label to read, I took the labelled doses from a peer-reviewed review of the drug (Prakash and Goa, BioDrugs 1999, PMID 18031173). It describes a single intravenous 1 mcg per kg dose as a diagnostic test, and once-daily subcutaneous 30 mcg per kg at bedtime as effective in some prepubertal children. It reports transient facial flushing and injection-site pain as the most common adverse events. I did not read the original US package insert, so treat the exact label wording as unverified.

There is no approved adult regimen to anchor to. Adult use today is off label, usually through a compounded product that FDA has not approved as a finished drug. Sermorelin does not appear on FDA's Category 2 list of bulk substances with safety concerns; I did not verify its Category 1 status.

What the published adult trials gave

Three small studies show how far the research doses sit from the forum doses. The first is Corpas and colleagues (JCEM 1992, PMID 1379256). Ten healthy men with a mean age of 68 took GHRH 1-29 under the skin twice daily for 14 days, at 0.5 mg and again at 1 mg, in random order. Only the 1 mg dose raised 24-hour growth hormone and IGF-I significantly, to levels not different from young men.

The second is Vittone and colleagues (Metabolism 1997, PMID 9005976). Eleven healthy men aged 64 to 76 injected 2 mg nightly for 6 weeks. Nighttime growth hormone release rose, but IGF-I did not change, body composition measured by DEXA did not change, and the authors reported no significant adverse effects. Two of six strength measures improved, and the authors concluded single nightly doses look less effective than several a day.

The third is Khorram and colleagues (JCEM 1997, PMID 9141536), and it comes with a warning: the drug was a modified analog, [Nle27]GHRH-(1-29)-NH2, not sermorelin itself, so do not copy its numbers. It gave 19 adults aged 55 to 71 a dose of 10 mcg per kg nightly for 16 weeks. IGF-I rose within 2 weeks and drifted back toward baseline by 16 weeks, and lean mass rose in men only. Some dosing pages blend this analog with sermorelin.

Put together: the highest trial doses (1,000 to 2,000 mcg per injection) are 2 to 10 times the 200 to 500 mcg that clinics usually prescribe, and even at 2 mg nightly one study saw no IGF-I change.

Reconstitution math: mcg, mL and units

Sermorelin comes as a freeze-dried powder mixed with bacteriostatic water, and the water volume sets the concentration. The formula is dose in mcg divided by concentration in mcg per mL gives mL, and mL times 100 gives units on a U-100 insulin syringe.

Units = dose in mcg divided by concentration in mcg per mL, times 100. A 5 mg vial holds 25, 16 or 10 doses at 200, 300 or 500 mcg. Checked by hand; confirm against your own vial.

Worked example: a 5 mg vial is 5,000 mcg. Add 2 mL of water and you get 2,500 mcg/mL. A 300 mcg dose is 300 divided by 2,500, which is 0.12 mL, or 12 units. The same vial holds about 16 doses of 300 mcg. If you instead add 1 mL, the concentration doubles to 5,000 mcg/mL and the same 300 mcg is only 6 units, which is easy to get wrong when you switch between vials.

Two Reddit comments match this math: 10 units from a 5 mg vial in 1 mL for 500 mcg, and 0.1 mL per 100 mcg from a 2 mg vial in 2 mL. A third said the prescribed dose was 33 units from a vial described as 0.9 mg in 10 mL, which works out to roughly 30 mcg if taken literally, so something was misreported. Compare doses in micrograms, not units, because a unit means nothing without the concentration.

If you hold a 10 mg research vial, the same method applies: 2 mL of water gives 5,000 mcg/mL and a 300 mcg dose of 6 units. Timing, frequency and cycle length

Nearly everything reported is a bedtime injection. In the Reddit data, 9 first-hand accounts said nightly or at bedtime, one said 5 days on and 2 days off, one said 10 nights a month, and one injects in the morning alongside two other peptides because nighttime use gave very vivid dreams. The rationale is to amplify the nighttime growth hormone pulse, and several leave a couple of hours after eating. No trial I found compared timing or meal gaps.

Cycle length is mostly habit. Commenters mention a planned 6-month course, a 3-month prescribed course, 8 weeks on and 2 off, 6 weeks, and one person who used it nearly every day for a year. These are routines, not evidence. The trials above ran 14 days to 16 weeks.

On when anything shows up, the accounts disagree. One person felt less winded within 2 days, another reported better sleep after 2 days, one after about 3 weeks, and one wrote that effects came late and quietly: "Probably nothing for 90 days, then at 6 months you're in a rhythm." (a r/Sermorelin commenter, Jan 2024). Another saw nothing at 2.5 months.

What Reddit users report taking

I read 407 scraped items and kept the first-hand ones: 38 comments and posts from 5 subreddits across 6 threads, mostly r/Sermorelin (31). Of those, 20 gave some dose figure and 12 gave a clear per-injection amount in mcg. That is a small, self-selected sample of people who chose to post, so read it as anecdote. Vendor pitches, price-shopping, bots and other compounds were dropped.

The most common current amounts were 500 mcg (4 accounts, one of them a sublingual tablet) and 100 mcg (3). The middle of the 12 is about 350 mcg. Ramping was common: several started at 100 to 250 mcg and moved up to 300 to 600 mcg over weeks. One commenter said anything over 200 mcg is wasted, which is an opinion, not data.

Four more accounts gave only a volume from a prescribed vial (0.1 cc, 0.2 mL, 0.3 mL and 0.33 mL), which cannot be converted without the vial strength. Three posts were unclear or implausible: one equated 10 units with 3 mg, one gave 15 mg in 0.5 mL, and one wrote a dose of 0.9 mcg a day with a hedge. I treat those as errors or confusion, not as doses. Another commenter asked whether 2 mg nightly would be fine with slow titration, having read that some people go that high; that is a number from forums, not from a clinician.

What people said happened is mixed. Of the 38, 7 reported better sleep, 5 more energy, endurance or less brain fog, 5 better muscle tone or recovery, and 5 little, late, or faded effect. Ten reported something unwanted: sore, itchy red welts at the injection site (2), vivid dreams (2), insomnia or worse sleep (2), swelling or pain in the hands and feet (1), allergic sensitivity that ended use (1), and a first dose of 200 mcg that brought a hot flash and fatigue (1):

"My initial dose was 200mcg, and I immediately got a hot flash that last about an hour along with EXTREME fatigue" (a r/Sermorelin commenter, Sep 2023).

The dreams complaint came with a workaround: "Taking it at night gives me craaazzzyyyy dreams" (a r/Sermorelin commenter, May 2026), said by a user who moved the injection to morning. One commenter described an emergency room visit with vomiting and said a doctor attributed it to antibodies; that is a single secondhand account and I could not verify it. What changes the dose

These are the variables that appear in sources, none of which can be turned into a personal number here.

Age and baseline IGF-I. A GHRH signal only helps if the pituitary can answer. Clinicians check IGF-I before and during treatment, which is why a prescriber, not a chart, should pick the number.

Dose and frequency. Corpas suggests larger or more frequent doses move hormone levels more, and Vittone found 2 mg once a night did not change IGF-I in older men.

Product form. Injection is the studied route. The tablet in the Reddit data (500 mcg sublingual) has no trial behind it that I found, and the user who tried it said six weeks produced no big difference. Sublingual and nasal products have no verified dose equivalence to injection.
