Tirzepatide (Zepbound) — the strongest shot

Tirzepatide is a dual GLP-1 and GIP agonist, sold as Zepbound for weight loss and Mounjaro for type 2 diabetes — the same molecule in two boxes. It is the most effective injection approved in 2026: 20.9% mean weight loss at 15 mg over 72 weeks in SURMOUNT-1, and in the only head-to-head trial, SURMOUNT-5, tirzepatide beat semaglutide 20.2% vs 13.7%. Semaglutide (Wegovy) — the proven all-rounder

Semaglutide is a GLP-1 agonist sold as Wegovy for weight loss and Ozempic for diabetes. Wegovy delivered 14.9% mean weight loss at 2.4 mg over 68 weeks in STEP 1, and it carries something tirzepatide is still gathering data on: proven cardiovascular protection (a 20% reduction in major cardiac events in SELECT) and kidney protection (FLOW). It is a daily shot rather than weekly, and the numbers are smaller: 8.0% mean weight loss over 56 weeks in the SCALE trial. Wegovy and Zepbound have largely overtaken it in real-world use. Saxenda's remaining roles are adolescent weight management (approved for ages 12+) and markets where weekly drugs are not covered. Generic liraglutide is now available in some places, which has pushed the price down.

Compounded semaglutide and tirzepatide — the budget route

Compounded versions are custom-mixed by licensed pharmacies rather than made by Novo Nordisk or Lilly. During the 2022–2024 shortages they were widely available; after the FDA declared the tirzepatide and semaglutide shortages resolved, large-scale compounding of straight copies was restricted. What to expect: side effects and injecting

All of these drugs share the same GI-heavy side-effect profile — nausea, diarrhea, constipation, and vomiting are the most common, usually worst during dose increases and easing over time.
